After the Doctor's near deathbed confession of his love for Seven, she seeks advice from Tom on how to return their relationship to friendship. Its simple really, Tom realizes, just make Seven unavailable...by having her date someone else. Chakotay has been pining after the Captain for too long, and its time he got back in the game, he reasons.
But the scheme brings up unanticipated feelings for two members of Voyager...and they begin to realize that you can't move past someone who will always be your home. J/C
